RF microneedling

Morpheus8 · Sylfirm X · Potenza · Secret Duo · Secret RF

Morpheus8 is a celebrity staple internationally; Sylfirm X and Potenza dominate searches in Singapore and Korea.

Search interest: about 1,400 searches a month in Singapore for “sylfirm x” (Ahrefs, October 2026).

The verdict Vetted

The tissue plane, not the device, decides whether you tighten skin or remodel fat. The brand war is a distraction from that.

Reviewed against the Aesthetics International standard. Category: Energy devices.

Does RF microneedling work?

Vetted. The tissue plane, not the device, decides whether you tighten skin or remodel fat. The brand war is a distraction from that. Good evidence for acne scars, pores and mild to moderate laxity, with meta-analyses supporting RF microneedling as comparable to fractional lasers for scars with less downtime in darker skin.

What it is

Insulated or non-insulated microneedles deliver radiofrequency energy at a chosen depth below the surface. Heat is delivered where it is wanted — in the dermis for texture and laxity, or at the septal plane for fat — while the epidermis is largely spared.

How it works

Depth, energy, pulse duration and needle insulation decide what the treatment does. Shallow passes remodel the dermis for pores, scars and fine laxity. Deeper passes in insulated mode reach fat and fibrous septa, which is how the same platform is used for submental fullness and body contouring. Brands differ in needle design and pulse control; the plane decides the result.

What the evidence says

Good evidence for acne scars, pores and mild to moderate laxity, with meta-analyses supporting RF microneedling as comparable to fractional lasers for scars with less downtime in darker skin. Evidence for fat reduction is device- and protocol-dependent. Over-deep, over-hot passes have produced fat loss and contour change on the face — the risk that follows aggressive 'tightening' marketing.

What the studies show

  1. systematic review

Pooled randomised trials found fractional CO2 somewhat more effective for acne scars and RF microneedling better tolerated, so the choice should be individualised to skin type and downtime tolerance.

Fractional CO2 Laser Versus Micro Needling Radiofrequency for Post Acne Scarring: A Meta-Analysis of RCTs. — J Cosmet Dermatol, 2026. PMID 41821146.

  2. systematic review

A network meta-analysis ranked RF microneedling among the effective energy options for atrophic scars, with a favourable side-effect profile relative to ablative lasers.

Comparative efficacy and safety of six photoelectric therapies for the atrophic acne scars: A network meta-analysis. — Indian J Dermatol Venereol Leprol, 2023. PMID 37067138.

  3. systematic review

RF microneedling showed benefit for acne scars, laxity, pores and several other indications, with study quality and protocols varying widely between devices.

Effectiveness of Radiofrequency Microneedling in the Treatment of Dermatological Conditions: A Systematic Review. — Aesthetic Plast Surg, 2026. PMID 42047762.

Questions

Morpheus8 or Sylfirm X — which is better?
They differ in needle design and pulse control, and each has indications it suits. The physician's choice of depth and energy for your indication matters more than the badge.
Can RF microneedling cause fat loss in the face?
It can when used too deep and too hot in thin areas. That is a planning error, and the reason depth is set per zone rather than per device.
How does it compare with CO₂ laser for scars?
Comparable for many scar types with less surface downtime, which makes it attractive in darker skin. Deep ice-pick scars still favour ablative point-shot work.
