What it is
Ultherapy is microfocused ultrasound with visualisation (MFU-V): focused ultrasound heats small points in the dermis and the SMAS layer at 1.5, 3.0 and 4.5 mm while an ultrasound image shows the physician which layer the transducer is sitting on. Prime is the newer platform with a larger screen, a redesigned handpiece and faster delivery; the energy and the depths are the same.
How it works
Each shot creates a small coagulation point that contracts and then stimulates collagen over two to three months. The imaging is the distinguishing feature: it lets the operator confirm the transducer is coupled and that 4.5 mm is actually reaching the SMAS rather than fat or bone. What varies between providers is the map — how many lines, at which depths, in which zones — and whether the operator reads the image or just counts shots. Prime changes how comfortably and consistently the energy is delivered, not what it does.
What the evidence says
Two systematic reviews with meta-analysis (2023 and 2025) found consistent, modest improvement in facial and neck laxity with high satisfaction, but noted that most trials are small, unblinded and often industry-supported. A 2025 systematic review of adverse events found that serious complications are uncommon and that fears of MFU-V compromising a later facelift are not supported by published data. Evidence specific to Prime is so far a pilot case series from Asia-Pacific; its claims rest on the older platform's record. The social complaint of facial fat loss appears in case reports rather than trials and is linked to depth selection in thin faces.
