Thermage vs Ultherapy vs HIFU

The three names people compare most are not three versions of one thing. Thermage is monopolar radiofrequency that heats the dermis broadly; Ultherapy and HIFU are focused ultrasound that coagulates points in deeper tissue. The question is not which is better but which layer your face needs treated.

Thermage vs Ultherapy vs HIFU: which suits whom?

Radiofrequency for skin quality and mild laxity across the whole face and neck; focused ultrasound for deeper descent along the jawline and brow; both in sequence when a face has both problems. Brand matters less than energy delivery, operator and selection.

By the numbers

  1. systematic review

Pooled data from 13 studies found measurable improvement in facial laxity and high satisfaction after microfocused ultrasound, with mostly transient side effects.

From the Korean HIFU variants review — A Systematic Review and Meta-Analysis of the Clinical Efficacy and Patients' Satisfaction of Micro-focused Ultrasound (MFU) Treatment for Facial Rejuvenation and Tightening., Aesthetic Plast Surg, 2023. PMID 37198297.

  2. cohort

A prospective study of 30 Asian patients with skin types III to V reported blinded-rater improvement in lower-face laxity at three and six months after a single treatment, with no serious adverse events.

From the Monopolar radiofrequency review — Efficacy and Safety of Monopolar Radiofrequency for Treatment of Lower Facial Laxity in Asians., Dermatol Ther (Heidelb), 2022. PMID 36166188.

  3. cohort

In a prospective trial of 15 subjects, two sessions improved brow, submental and neck laxity on blinded photographic grading at three and six months, with transient redness and swelling.

From the Sofwave review — Efficacy and safety of high-intensity, high-frequency, non-focused ultrasound parallel beams for facial skin laxity., J Cosmet Dermatol, 2024. PMID 38031530.

Questions

Is Ultherapy better than Thermage?
Neither is better in general. Ultherapy targets deeper descent in points; Thermage treats skin quality and laxity broadly. Faces with both problems often do best with both, planned in sequence.
Is HIFU the same as Ultherapy?
Ultherapy is one brand of micro-focused ultrasound with real-time imaging and the most independent evidence; other HIFU platforms use the same principle with different transducers and less published data.
How long do results last?
Published follow-up for both runs twelve to twenty-four months, with collagen response building over three to six months.
