Botox vs filler

One relaxes muscle, the other adds volume. Lines made by movement belong to the first; hollows, folds and lost structure belong to the second. The commonest mistake in aesthetic medicine is treating one problem with the other's tool.

Botox vs filler: which suits whom?

Neuromodulators for dynamic lines, masseter bulk and some neck bands; hyaluronic acid filler for structural volume in small amounts; biostimulators when a slower, more diffuse volume effect is wanted. Both are prescription-only medical treatments in Singapore, decided at consultation.

By the numbers

  1. systematic review

The world literature to 2018 recorded 146 new cases of filler-related vision loss, most after injection of the nose, glabella and forehead; HA was the commonest filler involved and few cases recovered vision.

From the Hyaluronic acid dermal fillers review — Update on Avoiding and Treating Blindness From Fillers: A Recent Review of the World Literature., Aesthet Surg J, 2019. PMID 30805636.

  2. systematic review

Across 28 studies masseter thickness fell by roughly a quarter at three months regardless of the number of injection points, with high heterogeneity and bias.

  3. systematic review

Ten small studies (153 participants) reported improvements in sebum, pore size, erythema and texture but not hydration, and none reached the required sample size for a firm conclusion.

Questions

Which is better for wrinkles, Botox or filler?
Lines that appear with expression: neuromodulator. Lines present at rest from volume loss: filler or resurfacing. Many etched lines need both over time.
Can you have Botox and filler together?
Yes, commonly in one visit, in different areas or layers.
Which lasts longer?
Filler, by months; but filler carries the larger risks and the more visible mistakes.
