What it is
Hyaluronic acid fillers are gels of cross-linked hyaluronic acid, a sugar molecule found naturally in skin, injected to restore volume, support structure or smooth lines. They differ in firmness, cohesivity and how much water they draw, which is why the same brand family has a dozen products for different depths and areas. Unlike biostimulators, their effect is immediate and they can be dissolved with hyaluronidase.
How it works
The gel occupies space where fat or bone has receded — the midface, temples, chin, jawline, lips — and, placed deep on bone, can project or lift adjacent tissue. It integrates over days, draws some water, and is gradually broken down over roughly six to eighteen months depending on product, site and movement. Placement plane and product choice decide whether a result looks like structure or like filler.

What the evidence says
A long record of randomised, controlled, blinded trials supports HA filler for nasolabial folds, midface volume and lips, with results persisting one to two years in the pivotal midface study. Meta-analyses in 2022 and 2024 confirm efficacy for the nasolabial fold and show lidocaine-containing products are as effective and more comfortable. The serious complication is vascular occlusion — filler blocking an artery — which can cause skin loss and, rarely, blindness; world literature reviews count hundreds of cases, concentrated in the nose, glabella and forehead. High-dose pulsed hyaluronidase protocols exist for this emergency, which is the main reason HA, not an irreversible filler, is the default choice.
