Lip lift surgery
Bullhorn lip lift · shortening the philtrum
The lip lift moved from a facelift add-on to a stand-alone TikTok and Instagram procedure in the late 2010s, sold to younger patients as the lasting alternative to lip filler.
In a long-philtrum face it is an elegant operation; in a short one it is a scar and a regret. Measure first.
Reviewed against the Aesthetics International standard. Category: Surgery.Does Lip lift surgery work?
Established. In a long-philtrum face it is an elegant operation; in a short one it is a scar and a regret. Measure first. A systematic review found only retrospective cohorts and case series, all reporting improved lip proportions and satisfaction maintained for one to five years, with no randomised data.
- Verdict
- Established Long clinical track record. Results depend on selection and technique more than on the device.
- Evidence base
- 3 PubMed-indexed studies cited below, including systematic reviews.
- Suits
- A long philtrum (roughly 15 mm or more), a thin upper lip that hides the teeth at rest, and a patient who accepts a visible scar under the nose.
- Not for
- Short philtrums, gummy smiles, keloid-prone or very young skin, and anyone whose real wish is volume, which filler gives reversibly.
- In Singapore
- A surgical procedure performed by plastic surgeons in licensed facilities in Singapore.

What it is
A surgical excision of a strip of skin just under the nose, shaped like a bull's horn, which lifts the upper lip, shortens the philtrum, everts the vermilion and shows more upper teeth at rest. Variants release the tissue more deeply or anchor it to the nasal base.
How it works
The philtrum lengthens with age and the upper lip thins and turns inward, hiding the teeth. Removing a measured strip of skin — a few millimetres decide the result — rotates the lip outward. Unlike filler it adds no volume; it changes proportion. The scar sits in the shadow of the nostril sill and is the trade-off: once skin is gone, a lip lifted too far cannot be lengthened again.
What the evidence says
A systematic review found only retrospective cohorts and case series, all reporting improved lip proportions and satisfaction maintained for one to five years, with no randomised data. A 2026 evidence-based review of lip lift techniques and a large single-surgeon series of a deep-plane variant describe refinements that reduce scar widening and relapse. Complications in the literature are scar visibility, asymmetry, nostril distortion, incomplete lip closure and over-shortening, with revision options limited. Candidate selection — a long philtrum with hidden teeth — predicts satisfaction more than technique.

What the studies show
- systematic review
Four retrospective studies showed improved lip aesthetics after subnasal lip lift with results maintained for one to five years; no randomised trials exist.
Impact of subnasal lip lift on lip aesthetic: a systematic review. — Oral and Maxillofacial Surgery, 2023. PMID 35253108.
- systematic review
Reviewed lip lift techniques and complications and set out selection criteria and conservative excision as the main safeguards.

- case series
A large single-surgeon series of a deep-release lip lift reported durable results and low revision rates with attention to scar placement.

Who it suits
Suits

Alternatives
The two reviewed treatments most often weighed against Lip lift surgery, on the same standard.
| Lip lift surgery | Hair transplant | Blepharoplasty | |
|---|---|---|---|
| Verdict | Established | Established | Established |
| What it is | A surgical excision of a strip of skin just under the nose, shaped like a bull's horn, which lifts the upper lip, shortens the philtrum, everts the vermilion and shows more upper teeth at rest. | Moving your own permanent hair follicles from the back and sides of the scalp to thinning areas. | Upper blepharoplasty removes excess skin, and sometimes a strip of muscle and fat, from the upper lid to open the eye and restore a visible lid platform. |
| Evidence base | 3 PubMed-indexed studies cited below, including systematic reviews. | 2 PubMed-indexed studies cited below, including systematic reviews. | 4 PubMed-indexed studies cited below, including systematic reviews. |
| Suits | A long philtrum (roughly 15 mm or more), a thin upper lip that hides the teeth at rest, and a patient who accepts a visible scar under the nose. | Stable pattern hair loss with a good donor area, realistic goals, a settled plan for the hair that will keep thinning, and a surgeon who does the surgery. | True skin excess hooding the upper lid, fat bags with a hollow tear trough, or lids that obstruct vision; patients who accept a scar and a fortnight of bruising. |
| In one line | In a long-philtrum face it is an elegant operation; in a short one it is a scar and a regret. Measure first. | The question to ask any clinic is who holds the punch, because the donor area is the one thing you cannot grow back. | The decision is about diagnosis: a low brow, a hollow or a fluid problem each look like 'tired eyes' and only one of them is fixed with a blade. |
Risks
Visible or widened scar, asymmetry, nostril base distortion, over-shortening with incomplete lip closure or gum show, numbness, and limited options for correction.
In Singapore
A surgical procedure performed by plastic surgeons in licensed facilities in Singapore. Aesthetics International does not offer surgery; patients asking about it are assessed for philtrum length and what they actually want, which is often shape rather than lift.
