What it is
Upper blepharoplasty removes excess skin, and sometimes a strip of muscle and fat, from the upper lid to open the eye and restore a visible lid platform. Lower blepharoplasty repositions or removes under-eye fat and tightens skin through an incision inside the lid or just under the lashes. In East Asian patients, double-eyelid surgery creates or defines a crease and is a related but distinct operation.
How it works
The upper lid ages by skin excess and brow descent; the lower lid by fat prolapse, hollowing at the tear trough and skin laxity. The operation addresses the component present: skin excision for hooding, fat repositioning for bags, canthal support when the lower lid is lax. The commonest error is removing too much — fat from a lower lid that needed it repositioned, or skin from an upper lid whose real problem was a low brow.
What the evidence says
A 2025 meta-analysis of 12 randomised trials found upper blepharoplasty reduced dry-eye symptoms compared with before surgery, that removing a muscle strip did not improve results but raised the chance of incomplete lid closure, and that satisfaction was high across techniques. A 2025 systematic review of lower blepharoplasty across 36 studies reported low complication rates with no major visual complications, with lid malposition the main concern and most problems manageable conservatively or by revision. The evidence favours conservative tissue removal and treating the brow and tear trough as part of the plan.
