What it is

Single-hair follicular units taken from the scalp, usually above the ear or at the nape, and placed one by one into the brow to restore density and shape. It is a small operation under local anaesthetic, typically a few hundred grafts, done by hair-restoration surgeons.

How it works

Scalp hair keeps its own growth cycle, so transplanted brow hairs grow long and need trimming every week or two for life. The angle, direction and curl of each graft must mimic the brow's changing pattern, which is the whole art; a brow placed at the wrong angle looks like a hedge. Grafts shed in the first weeks and regrow over three to six months; some fail, and a second session is sometimes needed.

What the evidence says

A 2018 systematic review of 67 articles and 354 patients found that follicular unit techniques are standard, complication rates are low and outcomes are generally good, with burns and trauma the commonest reasons for surgery in the literature and cosmetic over-plucking increasingly common in practice. Series describe graft survival in the brow as lower than on the scalp and survival of fine single hairs as the limiting factor. Patient-reported satisfaction is high in reviews, but published long-term outcome data are sparse and there are no controlled trials.
