Hair transplant: FUE and DHI

Follicular unit extraction · 'Turkey hair transplant' trend

Package hair-transplant tourism to Istanbul, documented in airport photos of bandaged heads and 'what really happens' articles, has made FUE the most visible cosmetic surgery for men under forty.

Search interest: about 1,400 searches a month in Singapore for “hair transplant” (Ahrefs, October 2026).

The verdict Established

The question to ask any clinic is who holds the punch, because the donor area is the one thing you cannot grow back.

Does Hair transplant work?

Established. The question to ask any clinic is who holds the punch, because the donor area is the one thing you cannot grow back. A long surgical track record and a 2025 systematic review and meta-analysis of complications: pain, swelling and donor-site discomfort are common and self-limiting, scarring at the recipient site is the main late complaint, and rare donor-site arteriovenous fistulas have been reported with FUE punches.

Verdict
Established Long clinical track record. Results depend on selection and technique more than on the device.
Evidence base
2 PubMed-indexed studies cited below, including systematic reviews.
Suits
Stable pattern hair loss with a good donor area, realistic goals, a settled plan for the hair that will keep thinning, and a surgeon who does the surgery.
Not for
Diffuse unpatterned thinning, scarring alopecia, young men with early loss and an unknown final pattern, or anyone who expects the transplant to stop hair loss. Relocating hair does not stop the loss that made it necessary.
In Singapore
Hair transplantation in Singapore is surgery performed by registered medical practitioners in licensed premises under MOH guidelines.

What it is

Moving your own permanent hair follicles from the back and sides of the scalp to thinning areas. FUE harvests follicular units one by one with a small punch, leaving dot scars; the older strip method (FUT) removes a strip and leaves a line. DHI is FUE with the grafts loaded into an implanter pen and placed without pre-made incisions. It is surgery, done awake under local anaesthetic over several hours.

How it works

Transplanted hair keeps the genetics of the donor area and keeps growing, which is why the result is durable; the hair around it keeps thinning, which is why the plan matters more than the graft count. Hairline design, graft survival, donor management and the angle and direction of placement decide whether the result looks like hair or like a transplant. Medical treatment continues afterwards to protect the native hair.

What the evidence says

A long surgical track record and a 2025 systematic review and meta-analysis of complications: pain, swelling and donor-site discomfort are common and self-limiting, scarring at the recipient site is the main late complaint, and rare donor-site arteriovenous fistulas have been reported with FUE punches. Reviews comparing FUE with strip harvesting find trade-offs in scar pattern and donor yield rather than a winner. Head-to-head trials of DHI against standard FUE are scarce; it is a placement technique, not a different operation. The literature on tourism outcomes is mostly case reports of over-harvested donors and unnatural hairlines.

What the studies show

  1. systematic review

Across 45 articles hair transplantation was generally safe, with donor-site pain and swelling the commonest complications, recipient-site scarring the main late problem and rare donor arteriovenous fistulas linked to FUE punches.

Complications Following Hair Transplantation: A Systematic Literature Review and Meta-Analysis. — Aesthetic Plast Surg, 2025. PMID 40913181.

  2. review

FUE and strip harvesting each carry trade-offs in scarring, donor yield and graft handling that should be weighed patient by patient rather than by trend.

The pros and cons of follicular unit extraction (FUE) versus elliptical donor harvesting (FUT). — J Cosmet Laser Ther, 2022. PMID 36040012.

Who it suits

Alternatives

The two reviewed treatments most often weighed against Hair transplant, on the same standard.

What it isMoving your own permanent hair follicles from the back and sides of the scalp to thinning areas.Single-hair follicular units taken from the scalp, usually above the ear or at the nape, and placed one by one into the brow to restore density and shape.Upper blepharoplasty removes excess skin, and sometimes a strip of muscle and fat, from the upper lid to open the eye and restore a visible lid platform.

SuitsStable pattern hair loss with a good donor area, realistic goals, a settled plan for the hair that will keep thinning, and a surgeon who does the surgery.Permanently sparse brows from over-plucking, scarring, burns or thyroid-related loss that has stabilised, in someone willing to trim for life and wait six months.True skin excess hooding the upper lid, fat bags with a hollow tear trough, or lids that obstruct vision; patients who accept a scar and a fortnight of bruising.
In one lineThe question to ask any clinic is who holds the punch, because the donor area is the one thing you cannot grow back.Established and not reversible, which is exactly why the shape should be chosen for your face and not for this year's brow. The decision is about diagnosis: a low brow, a hollow or a fluid problem each look like 'tired eyes' and only one of them is fixed with a blade.

Risks

Swelling, numbness, folliculitis, poor growth, visible donor thinning, unnatural hairlines, and the rare serious complications of any surgery done at scale with inadequate medical oversight.

In Singapore

Hair transplantation in Singapore is surgery performed by registered medical practitioners in licensed premises under MOH guidelines. Aesthetics International does not offer surgery; this is an editorial review. Non-surgical hair protocols such as Dr Sin Yong's H2LT address the follicles that remain.

Questions

FUE or DHI?
DHI is FUE with an implanter pen. It can give denser placement in skilled hands; it does not change survival or outcome by itself.
Is a Turkey hair transplant safe?
Many are done well. The problems reported are over-harvesting, technician-led surgery and poor hairline design, which no price can undo. Ask who performs the extraction and placement.
Will I need medicine after a transplant?
Usually yes, to protect the native hair around the grafts. That conversation happens with your doctor before surgery.
