What it is

Injection of hyaluronic acid or other fillers across the forehead to create a smoother, more rounded contour or to soften a flat or sloping profile. It is a volumising treatment, not a wrinkle treatment; horizontal lines are usually treated with neuromodulators.

How it works

Filler is placed deep, typically in the plane just above the bone beneath the muscle, often with a cannula, and moulded to a smooth curve. Relatively large volumes are needed. The forehead's supratrochlear and supraorbital arteries connect with the eye's circulation, which is the central safety concern.

What the evidence says

Anatomical studies in 2022 and 2025 define safer injection planes and pinch techniques for forehead filler. The 2019 world-literature review of filler blindness lists the forehead among the sites most often associated with vision loss, alongside the nose and glabella. Outcome studies are small and aesthetic; there are no controlled trials showing benefit that outweighs the vascular risk for routine contouring.

Is forehead filler safe?
It carries a higher vascular risk than many filler sites because forehead arteries connect to the eye's circulation. Risk is reduced, not removed, by technique and reversible filler.
