Established. Patience beats escalation for most warts. A 2012 Cochrane review of topical treatments for cutaneous warts found salicylic acid more effective than placebo, with cryotherapy of similar efficacy and no clear advantage between them.

What it is

Warts are benign skin growths caused by human papillomavirus infecting the outer skin. Common warts appear on hands and fingers, plantar warts on the soles, flat warts on the face. Many clear on their own within two years; treatment speeds clearance or removes warts that hurt, spread or bother.

How it works

Salicylic acid softens and peels the wart over weeks; cryotherapy freezes it with liquid nitrogen in a clinic, repeated every two to three weeks; resistant warts may be treated with laser, cantharidin, immunotherapy or surgery. None kills the virus in surrounding skin, which is why recurrence is common.

What the evidence says

A 2012 Cochrane review of topical treatments for cutaneous warts found salicylic acid more effective than placebo, with cryotherapy of similar efficacy and no clear advantage between them. The 2011 EVerT randomised trial in the BMJ found cryotherapy and salicylic acid equally effective for plantar warts, with clearance in only about one in seven at twelve weeks — a reminder that verrucae are stubborn.

What the studies show

  1. systematic review

Salicylic acid was better than placebo; cryotherapy was of similar efficacy to salicylic acid, with no clear advantage of one over the other for common warts.

  2. RCT

In 240 participants with plantar warts, clearance at 12 weeks was similar with cryotherapy and salicylic acid, at about 14 per cent in each group.

Cryotherapy versus salicylic acid for the treatment of plantar warts (verrucae): a randomised controlled trial. — BMJ, 2011. PMID 21652750.

What it isWarts are benign skin growths caused by human papillomavirus infecting the outer skin.Milia are tiny keratin-filled cysts just beneath the skin surface, one to two millimetres across, white or yellowish, typically around the eyes and cheeks.Skin tags are soft, benign, skin-coloured or slightly darker growths on a narrow stalk, from a millimetre to a centimetre, in the folds — neck, armpits, groin, eyelids.

SuitsWarts that are painful, spreading, cosmetically unwanted or persistent — starting with salicylic acid or clinic cryotherapy, with a doctor for facial, genital or resistant warts.Primary milia around the eyes and cheeks in adults, secondary milia after a procedure, and anyone who wants them gone in one visit rather than hoping skincare will work.Tags that catch, rub or bleed, or that the person simply wants gone, removed by a doctor in a single visit; people with many tags who also want a metabolic check.
In one linePatience beats escalation for most warts. The error is spending months on products that cannot reach a cyst under intact skin. The only judgement needed is confirming it is a tag; after that, it is scissors and a minute.

Risks

Blistering, pain and temporary pigment change after cryotherapy; irritation from salicylic acid; scarring with surgery or aggressive laser; recurrence.

In Singapore

Wart treatment in Singapore is offered by GPs, dermatologists and aesthetic doctors; salicylic acid is available in pharmacies. Genital warts need a doctor and are managed differently.

Questions

Is freezing or salicylic acid better for warts?
Trials and a Cochrane review find them similarly effective; cryotherapy is quicker per session but more painful.
Does laser remove warts?
Lasers are used for resistant warts with variable results; they are not first line.
Why do warts come back?
The virus persists in surrounding skin; new warts can appear after treatment.
Will warts go away on their own?
Many common warts clear within two years without treatment.
How many cryotherapy sessions do warts need?
Often several, two to three weeks apart; plantar warts may need more.
Can I treat facial warts at home?
No. Facial skin scars and pigments easily; see a doctor.
