What it is
A cosmetic tattoo of the lips: a machine with a fine needle deposits sheer pigment through the vermilion to even out colour, sharpen the border and give the illusion of fullness. It is a tattoo of mucosa, which heals, fades and reacts differently from skin.
How it works
Pigment in the lip dermis gives colour that looks dark and swollen for a week, peels, 'ghosts' almost away, then returns softer over four to six weeks. The lip turns over fast and is well vascularised, so colour fades within one to three years, sometimes unevenly, and must be refreshed. Needling the lip reactivates herpes simplex in anyone who carries it, which most adults do, so antiviral cover prescribed by a doctor is standard before treatment.
What the evidence says
There are no trials. The literature is reviews of cosmetic tattooing complications: early herpes reactivation, infection and healing problems, and late colour shift, pigment migration beyond the border, allergic and granulomatous reactions, and pigmented macules that persist after the tint fades. A 2022 systematic review of systemic infections after tattoos and permanent make-up found 17 reports in a decade, including septic shock and one death, all linked to hygiene. Interaction with lip filler is described: pigment can distort when the lip is later filled or anaesthetised.
