Dark circles: pigment, vessels or shadow
periorbital hyperpigmentation · infraorbital dark circles · tear trough shadow
Dark eye circles are one of the most searched cosmetic concerns in Singapore, and the treatments sold for them — creams, lasers, fillers, boosters — rarely say which kind of dark circle they treat.
Search interest: about 450 searches a month in Singapore for “dark eye circles” (Ahrefs, October 2026).
Brown is pigment, blue is vessels, grey is shadow; the money is wasted when the treatment is chosen before the diagnosis.

Established. Brown is pigment, blue is vessels, grey is shadow; the money is wasted when the treatment is chosen before the diagnosis. A 2021 systematic review of periorbital hyperpigmentation treatments found modest benefit from topical agents, chemical peels, lasers, PRP and fillers, with study quality generally low and benefit dependent on matching the treatment to the cause.
- Verdict
- Established Long clinical track record. Results depend on selection and technique more than on the device.

- 4 PubMed-indexed studies cited below, including systematic reviews.
- Suits
- Anyone with dark circles who wants to know which kind they have before spending on creams, lasers or filler; pigmented circles in Asian skin respond well to a sun-protection-plus-pigment plan.
- Not for
- Under-eye filler as a first move for pigment or vascular circles; aggressive lasers on thin periorbital skin; anyone expecting a single cream to change structure.

What it is
Dark circles have three common causes, often mixed. Pigmented circles are brown, from excess melanin, common in Asian skin. Vascular circles are blue-purple, from visible vessels under thin skin. Structural circles are shadows cast by a tear-trough hollow or eye-bag bulge. The treatment that works depends on which dominates.
How it works
Pigment is treated with sun protection, topical lightening agents and pigment lasers at gentle settings. Vascular circles improve with skin-thickening treatments and sometimes vascular lasers. Structural shadows need volume (filler, carefully) or surgery for bags. Lifestyle factors — sleep, allergies, rubbing — affect all three.

What the evidence says
A 2021 systematic review of periorbital hyperpigmentation treatments found modest benefit from topical agents, chemical peels, lasers, PRP and fillers, with study quality generally low and benefit dependent on matching the treatment to the cause. Reviews in 2016 and 2018 classify dark circles by aetiology and recommend cause-specific treatment. A 2024 framework from the Aesthetic Surgery Journal proposes stepwise assessment of the tear trough and lid–cheek junction before choosing filler or surgery.

What the studies show
- systematic review
Topical agents, chemical peels, lasers, PRP and fillers each improved periorbital hyperpigmentation in some studies, but evidence quality was low and results depended on aetiology.
Treatments of Periorbital Hyperpigmentation: A Systematic Review. — Dermatol Surg, 2021. PMID 32740208.
- review
Dark circles arise from pigmentation, vascularity, thin skin and structural shadowing; treatment should follow the dominant cause.

- review
Pigmented, vascular and structural types respond to different treatments, and mixed types need combined approaches.

- clinical framework
A stepwise assessment separating skin colour, volume and fat prolapse guides whether filler, energy treatment or surgery is appropriate.

| Dark circles | Pico laser | PDRN and polynucleotides | |
|---|---|---|---|
| Verdict | Established | Vetted | Established |
| What it is | Dark circles have three common causes, often mixed. | A picosecond laser delivers energy in pulses a thousand times shorter than the nanosecond lasers that came before it. | Polynucleotides are DNA fragments, usually purified from salmon or trout gonads, injected into the dermis in micro-droplets. |
| Evidence base | 4 PubMed-indexed studies cited below, including systematic reviews. | 4 PubMed-indexed studies cited below, including systematic reviews. | 3 PubMed-indexed studies cited below, including systematic reviews, randomised trials. |
| Suits | Anyone with dark circles who wants to know which kind they have before spending on creams, lasers or filler; pigmented circles in Asian skin respond well to a sun-protection-plus-pigment plan. | Sun spots, freckles, post-acne marks, tattoo ink, uneven tone in Asian skin when energy is set conservatively. | Thin, crepey under-eye skin; dull, inflamed or acne-prone skin; post-procedure recovery; people who want quality rather than volume. |
| In one line | Brown is pigment, blue is vessels, grey is shadow; the money is wasted when the treatment is chosen before the diagnosis. | The variable that matters is depth, not brightness, and pigment already deposited is only half the problem; the inflammation producing it is the other half. | The injected courses are a sensible part of a skin plan; the salmon-sperm facial is mostly the microneedling doing the work. |
Risks
Post-inflammatory hyperpigmentation with heat-based lasers; filler swelling and Tyndall blue tint; irritation from strong topicals on thin eyelid skin.
In Singapore
Dark circle treatments in Singapore range from pharmacy creams to doctor-performed lasers and fillers. Because pigmented circles are common in Asian skin and heat risks darkening, conservative laser settings and sun protection are the default.
