Acne scar types: ice pick, boxcar and rolling
atrophic acne scars · scar classification · 'types of acne scars'
Most people researching acne scars start by trying to name what they see in the mirror, and 'types of acne scars' is searched steadily in Singapore before people book a consultation.
Search interest: about 250 searches a month in Singapore for “types of acne scars” (Ahrefs, October 2026).

What it is
Atrophic acne scars — the depressed kind — are classified into three main shapes. Ice-pick scars are narrow (under 2 mm), deep and sharply defined. Boxcar scars are wider, with steep edges and a flat base. Rolling scars are broad, shallow undulations caused by fibrous bands tethering the skin to deeper tissue. Raised hypertrophic and keloid scars are a separate group, and dark or red marks are not scars at all.
How it works
Each shape needs a different tool. Ice-pick scars are reached with TCA CROSS or punch techniques; boxcar scars respond to fractional resurfacing, RF microneedling and, for deep ones, punch elevation; rolling scars need subcision to release the tether, often followed by resurfacing or volume. Most faces have a mixture, which is why combined, staged protocols outperform any single device.

What the evidence says
The widely used classification by Jacob and colleagues, summarised in a 2010 review, divides atrophic scars into ice-pick, boxcar and rolling and links each to treatment. A 2020 review updates the classification and treatment options. Agreement between experts in classifying individual scars is only moderate, as a 2013 study showed, which is one reason a hands-on assessment in good light matters more than a photo.

| Acne scar types | Pico laser | RF microneedling | |
|---|---|---|---|
| Verdict | Established | Vetted | Vetted |
| What it is | Atrophic acne scars — the depressed kind — are classified into three main shapes. | A picosecond laser delivers energy in pulses a thousand times shorter than the nanosecond lasers that came before it. | Insulated or non-insulated microneedles deliver radiofrequency energy at a chosen depth below the surface. |
| Evidence base | 4 PubMed-indexed studies cited below, including systematic reviews. | 4 PubMed-indexed studies cited below, including systematic reviews. | 3 PubMed-indexed studies cited below, including systematic reviews. |
| Suits | Anyone with acne scars who wants to understand their scar mix before choosing treatment — the first step of any honest scar plan. | Sun spots, freckles, post-acne marks, tattoo ink, uneven tone in Asian skin when energy is set conservatively. | Acne scars, enlarged pores, early laxity, melasma-prone skin that cannot take ablative lasers, and selected body contouring when the plane is mapped first. |
| In one line | Most faces need all three, in order. | The variable that matters is depth, not brightness, and pigment already deposited is only half the problem; the inflammation producing it is the other half. | The tissue plane, not the device, decides whether you tighten skin or remodel fat. The brand war is a distraction from that. |
Risks
Misclassification leads to the wrong treatment and wasted sessions; that is the main risk this page addresses.
In Singapore
Acne scar treatment in Singapore is offered by doctors using fractional lasers, RF microneedling, subcision and TCA CROSS. Because Asian skin pigments easily after inflammation, plans are staged and conservative.
