What it is

Endermologie is motorised roller-and-suction massage of the skin and fat layer, developed in France in the 1980s. VelaShape adds infrared light and bipolar radiofrequency to the vacuum and rollers so the tissue is heated while it is kneaded. Both are offered in courses of six or more sessions for cellulite and circumference.

How it works

Suction and rolling mobilise interstitial fluid, stretch the connective tissue and increase local blood flow; heat from RF and infrared adds a mild, temporary contraction of collagen and may encourage fat metabolism. The visible effect is largely fluid shift and skin tone, which is why it is most convincing immediately after a session and fades over weeks. Nothing is removed and no septum is released.

What the evidence says

Mechanical massage has been studied for a quarter of a century: a 1999 randomised trial found no objective improvement in cellulite from Endermologie, and a 2009 series found circumference loss but improved cellulite grade in only a minority. VelaShape's manufacturer-supported studies report a few centimetres of circumference reduction and mild tightening after a course, measured by tape rather than imaging. The device is cleared in the US for temporary reduction of cellulite and circumference, and 'temporary' is the operative word.

Questions

Does VelaShape reduce cellulite?
Temporarily and mildly. Objective studies of mechanical massage show little lasting change in cellulite grade.
Why do before-and-after photos look so good?
They are taken minutes after suction and heat have moved fluid and flushed the skin. Photograph the same leg a week later.
Is Endermologie useful for anything?
It is a reasonable adjunct after liposuction for swelling and fibrosis, and a pleasant massage. It is not a contouring treatment.
