Picosecond and non-ablative fractional lasers help texture and the marks around scars with less downtime but smaller effect per session. Topical actives soften post-inflammatory marks but do not remodel a scar. In Asian skin the limiting factor is post-inflammatory hyperpigmentation, so energy settings and pre-treatment matter as much as the device. - [Pigmentation and melasma](https://aesthetics-international.org/concerns/pigmentation-and-melasma/): For lentigines and freckles, Q-switched and picosecond lasers have a long track record. For melasma, the evidence favours a maintenance approach: strict broad-spectrum sun protection, topical tranexamic acid, azelaic acid, retinoids and prescription lightening agents, with oral tranexamic acid under medical supervision in resistant cases; low-fluence laser toning adds modest benefit and carries a rebound and mottling risk when overused. Peels and picosecond lasers help selected cases. Supplements marketed as whitening pills have weak evidence. - [Eye bags and dark circles](https://aesthetics-international.org/concerns/eye-bags-and-dark-circles/): Fat prolapse is a surgical problem: lower blepharoplasty, often with fat repositioning, has the longest track record. A hollow tear trough with good skin can be softened with conservative hyaluronic acid filler, but under-eye filler is also the most regretted filler on the face because of swelling, blue tint and migration. Loose, crepey skin responds modestly to fractional lasers, radiofrequency and skin boosters. Pigmented dark circles are treated as pigmentation; vascular dark circles rarely improve with devices. Fluid-related puffiness responds to sleep, salt and allergy control more than to any treatment. - [Sagging skin and skin tightening](https://aesthetics-international.org/concerns/sagging-and-skin-tightening/): Monopolar radiofrequency and micro-focused ultrasound have the longest independent evidence for modest, measurable tightening, with results that depend on selection and energy delivery more than on brand. Threads reposition tissue temporarily and do not tighten skin. Biostimulatory injectables restore volume but cannot lift descended tissue. Deep-plane facelift remains the only treatment that repositions the deep layers durably. Devices marketed as equal to surgery are not. - [Hair loss and thinning](https://aesthetics-international.org/concerns/hair-loss/): The evidence-based core for pattern hair loss is prescription medicine (topical and oral options a doctor can discuss), with low-level laser therapy and platelet-rich plasma as adjuncts supported by randomised trials of modest size. Hair transplantation moves hair; it does not stop loss elsewhere. Scalp microneedling adds to topical therapy in trials. Exosome and most supplement claims rest on small or sponsored studies, and biotin helps only true deficiency. - [Cellulite and body contouring](https://aesthetics-international.org/concerns/cellulite-and-body-contouring/): Cryolipolysis and high-intensity electromagnetic stimulation have randomised and controlled data for modest, measurable fat and muscle changes in suitable candidates. Radiofrequency and ultrasound tighten skin modestly. For cellulite, the treatments with the most convincing evidence release the fibrous septae (targeted subcision); massage, suction and cream-based approaches show short-lived change. Injectable fat dissolvers outside the submental area are off-label and poorly studied. - [Double chin and jowls](https://aesthetics-international.org/concerns/double-chin-and-jowls/): Submental fat responds to cryolipolysis, deoxycholic acid injection and energy-based lipolysis, each with controlled data. Laxity responds to monopolar radiofrequency and micro-focused ultrasound. Masseter hypertrophy responds to neuromodulator injection. A retruded chin is corrected with filler or implant. Threads and most topical or at-home approaches have little evidence here. - [Wrinkles and fine lines](https://aesthetics-international.org/concerns/wrinkles-and-fine-lines/): Topical retinoids and daily sunscreen have the longest evidence for fine lines. Neuromodulators are the standard for dynamic lines. Fractional resurfacing, RF microneedling and hyaluronic acid skin boosters improve etched lines and texture with controlled data. Collagen supplements show small improvements in hydration and elasticity in industry-funded trials. Face taping, wrinkle patches and face yoga have little evidence beyond temporary effects. - [Pores, texture and dullness](https://aesthetics-international.org/concerns/pores-and-texture/): Retinoids and chemical peels reduce pore visibility and surface roughness in controlled studies. Non-ablative fractional lasers, picosecond fractional handpieces and RF microneedling improve texture with modest downtime. Hydrafacial-type treatments and skin boosters give short-lived hydration and glow. Dermaplaning, gua sha, ice rolling and most viral routines are cosmetic and temporary. Oral isotretinoin, a prescription medicine, changes oil output and is discussed only at consultation. - [Neck lines and 'tech neck'](https://aesthetics-international.org/concerns/neck-lines-and-tech-neck/): Hyaluronic acid skin boosters and low-dose neuromodulator injections into the platysma soften horizontal lines with case-series evidence. Fractional non-ablative lasers and RF microneedling improve texture and crepiness. Monopolar radiofrequency tightens mild laxity. Aggressive ablative resurfacing on the neck carries scarring risk and is used cautiously. Posture changes limit progression. - [Acne](https://aesthetics-international.org/concerns/acne/): The evidence-based foundation is topical retinoids and benzoyl peroxide for mild to moderate acne, with azelaic acid and niacinamide as adjuncts; oral antibiotics for a limited course in moderate inflammatory acne; hormonal treatment for adult female acne; and oral isotretinoin for severe, nodular or scarring acne, the one medicine with lasting remission. Procedures earn their place around the medicine: steroid injection for a painful cyst, chemical peels and light-based treatments for stubborn comedonal and inflammatory acne, and the 1726 nm lasers as a newer, device-based route to the sebaceous gland. Facials, 'detox' routines and most supplements do not change the course of acne. - [Moles, warts and skin tags](https://aesthetics-international.org/concerns/moles-warts-and-skin-tags/): Skin tags are removed by snip, cautery, cryotherapy or ablation with near-certain results; the medical literature concerns their link with insulin resistance rather than their removal. For moles, systematic reviews in 2026 support surgical shave or excision, which preserve histology and recur less, and caution against laser for any lesion not unequivocally benign because melanomas have been diagnosed late in laser-treated lesions. Warts respond to salicylic acid and cryotherapy as first-line treatment, with laser and immunotherapy for resistant cases; recurrence is common whatever the method. - [Redness and rosacea](https://aesthetics-international.org/concerns/redness-and-rosacea/): For rosacea, the evidence favours trigger management and sun protection, topical metronidazole, azelaic acid or ivermectin for bumps and pustules, oral low-dose doxycycline for inflammatory disease, and vascular lasers or IPL for background redness and visible vessels, where pulsed dye laser has the longest record. Barrier repair and stopping exfoliating acids resolve much 'redness' that was never rosacea. Niacinamide helps barrier and tone modestly.
